@Report of Medical Examination for the children at Matumaini Children's
Home and so called "Street Children" in Mbagathi District

Preface :
We have been supporting the Matumaini Children's Home, run by Ms. T. Kikumoto, since 1989. Last year Dr. Mori decided to visit Kenya and Matumaini, for getting the details about the life of children. He realized the children at home are quite healthy not only physically but mentally too. After he came back he discussed with the members of Ishinkai International Voluntary Movements(IIVM) about the medical workshop of IIVM at Matumaini, 1998. All of the members agreed to have such attempts , including the medical examination for the children at the home and those are living on the street. He asked to Professor H.O. Pamba, Dean , College of Health Sciences, University of Nairobi, giving us the acknowledgement of medical examination. Professor Pamba gave us the acknowledgement and also kindly arranged the cooperation of Professor Bwibo, Department of Pediatrics. SRL, Co.Ltd, leading company of laboratory medicine, decided to participate in this project sending the medial technician for the workshop . Medical workshop was held in between August 1-13,1998.

Method:
Workshop members : Medical staffs listed above and other volunteers Medical examination : Medical history, Physical examination, Urinalysis, Blood chemistry, HIV test were done for all candidates, except some children refusing the blood tests. Urinalysis was checked by Uro-LabstixR SG-L, Bayer-Sankyo Co.Ltd. Blood chemistry was examined by Fuji Drychem 3030,@Fuji@Medical SystemACo.Ltd. ( 6 items : total protein, AST, LDH, ALP, T-Cholesterol, BUN) HIV test was done by Simpified HIV detectable kit, Dynabott, Co.Ltd.

Details of the results of laboratory tests are listed in Fig. 4. Because of the limitation of schedule, the members of SRL were obliged to leave before the examination of Street group. This is why the results were only for Matumaini group.

The body measurements showed street children were somwhat smaller than those of home's children. Multiple lymph nodes swelling were seen in street group suggesting more frequent exposure to upper respiratory infections. I could not find any good reason for struma seen in home's children, although all of them have few chances to take the foods containing iodine, like seaweed.

Discussion :
Nutrition : As showed in Fig.2 & 3, nutrition of Matumaini group is well preserved as compared to Street group. Although their meals were mainly maze, beans and other vegetables, three times meals were more than enough from the view point of nutrition. The voluntary movement, like a charity pot, is essential to the street children.

HIV infection :
Since the number of children is not enough to evaluate the morbidity of HIV infection, we cannot make the conclusion. Informally says, one positive case per approximate forty persons is seen.

Conclusion :
The medical workshop in 1998 is thought to be a preliminary attempt. From 1999, more complete plan should be prepared in cooperation with the members of Faculty of Medicine, University of Nairobi.
